Terms and Conditions***

I, on behalf of my minor child named above, heirs, assigns, personal representatives, hereby expressly release, discharge, indemnify and hold harmless Paadapadmam Center for Performing Arts, LLC (PCPA)and its Directors and persons acting behalf of it, from all liabilities, claims, demands, actions, arising out of any activity or dance classes including the rehearsals or performances. I acknowledge that performance offered by PCPA bring with it risk including but not limited to falling, slipping, colliding with another student and causing any type of injury. The child is being enrolled for physical activities and confirm that the child is in good physical condition and there are no conditions that prevent the child from participating in physical/dance activities or program. 

 

I hereby grant permission for myself and/or my child to be photographed, videotaped, and/or interviewed during the course of the study and performances at PCPA. 

 

PCPA or any of its authorized agents, is hereby given consent for the publication, broadcast or use of the images or words for the purpose of promotion and publishing of its advertisements electronically or through social media. I/We hereby release Paadapadmam from any liability for use of such images or statements.
